A Credentialed Pharmacist is a consultant that has specialised in Medication Management. The Credentialed Pharmacist visits people on GP referral at home and provide education on medication, looks for any side effects and medication interactions this include both prescribed and non-prescribed medications and health care products the client takes or use. This forms a report that helps the Doctor and patient continue and optimise the usage of medication. A medication review also looks for medication that no-longer are needed and provide advise on how to stop the medication.
